Appointments
In order to make a counselling appointment we ask that you register first, by completing and returning a registration form along with a £5 registration fee. There are several ways to do this:
You can click here to complete our online form which is the quickest and easiest way to register. After submitting the form online, you will automatically be taken to our ‘To Donate’ page where you can pay the £5 Registration Fee with your own card.
You can phone us on 0151 737 2121 and ask us to post out a registration form and additional information. This can then be completed and returned in the freepost envelope that will be enclosed, along with a £5 registration fee in order to arrange the first appointment.
You can print out the registration form from our link below and then post it to us along with a £5 cheque payable to ‘Reach Merseyside Limited’. Please post this to our address at 85a Allerton Road, Liverpool L18 2DA.
You can download the registration form here: Word | PDF (print and handwrite) and then email it into us at [email protected]. The £5 registration fee can then be given via our to donate page. Please remember to fully complete the registration form and to indicate your level of donation per session after having considered our Fair Donation Principle which can be found on the ‘Donation Guidelines’ page.
Please note parental or guardian’s permission must be obtained in order to counsel anybody aged 17 and under. The appropriate consent form can be found here: Word | PDF (print and handwrite). This permission form has to be returned along with a registration form.
Answering your questions
Please contact us if you have any questions that would help you to decide if counselling at Reach would be right for you at this time. There is usually somebody available to answer the phones between 10am and 4pm Monday to Friday on 0151 737 2121. Outside of these hours, or if reception is busy, a message can be left on our answerphone, or you can email us.
What happens after registering?
After receiving a registration form, our Appointments Manager, matches new clients to the most suitable available counsellor within our team to enable progress. You will be offered an appointment time and location and this will be confirmed with all involved. We hope to offer an appointment date within 4 weeks of receiving a Registration Form.
